I always try to add credits to Copyright.txt when I check patches in
for people. I am 100% sure that there are gaps in that file, some that
we will never know about.
Though to be fair, it is in the directions for how to submit your
patch to make sure you add your name to Copyright.txt and add that to
the diff.
Credits.rtf I don't really touch, but what Eric said seems reasonable
to me.

Because the person who checked it in forgot, of course. We try to do
our best, and if people would follow the directions on the wiki, this
problem wouldn't happen as much.
