Hopefully this is your issue as well.  I found that the only times  
this happens to me is when I have Safari set in private browsing.   
Sometimes I engage private browsing for some reason or another and  
forget to turn it off, and a login to forums or trac or something is  
the fist indicator to me that I accidentally left it on.
